                                                                                                            effective in helping people create
                                                                                                                                            healthy lifestyle changes!

                                                                                                            This is an integrated, e-mail based health promotion program
                                                                                                            that takes only 30 minutes to begin and a few minutes once a week to
                                                                                                            get new health tips and track your progress!


                                                                                                            WHAT A GREAT OPPORTUNITY TO LINE UP ON THE ROAD TO WELLNESS!
                                                                                                             
                                                                                                            The Baseline Questionnaire includes the diet and physical activity health risk assessment that measures participants' current diet and
                                                                                                            activity levels.  It takes only about 30 minutes to start this life changing program.  After completing the questionaire, the program provides the participant with immediate feedback and recommends a "Big Goal" to work on
                                                                                                            for the first 12 week program cycle.  The current Big Goals in the system
                                                                                                            include one of the following known health improvement behaviors that most
                                                                                                            of us know we could use some help with:  
                                                                                                                1.  Increasing fruit, vegetable and fiber intake, 
                                                                                                                2.  Limiting sugar, carbs, saturated & trans fats and
                                                                                                                3.  Increasing physical activity.

                                                                                                            Then, Each week Alive! will send suggested goals for you to work on for the
                                                                                                            next week, that are personalized to your current diet, physical activity, and lifestyle. Through doing small, achievable steps, you move gradually to your target of improved diet and activity. After selecting your weekly goal, you are automatically taken to a personalized web page that provides you with the tips and tools you need to accomplish your goals.

                                                                                                            After each 12-week period, you will complete a short questionnaire online
                                                                                                            to see how well you are doing and to customize the best steps for you to
                                                                                                            work on next.  You may choose to work on one of the other Big Gaols, or continue working on the same one.  Thus, you can improve in one or more 
                                                                                                            critical health improvement areas!
